Questionnaire needs some love. This popular Moodle plugin has been in use since the early days of Moodle; close to 20 years. As such, it has collected a lot of technical debt. In order to continue to maintain this plugin effectively, it requires a refactoring of the code. This refactoring will:

Questionnaire 5.0.1 supporting Moodle 5.0 and 5.1 has been released. Details are in the “CHANGES” file. The release can be found on Github and the Moodle plugins database.

Questionnaire 4.4.0 supporting Moodle 4.4 and 4.5 has been released. Details are in the “CHANGES” file. The release can be found on Github and the Moodle plugins database.

There is a new release of the questionnaire that runs on 3.9, 3.10 and 3.11. Among other bug fixes, there is one security issue fixed. It is recommended that you upgrade as soon as possible.

This release provides a number of new features, improvements, and bugfixes. It supports Moodle 3.9, 3.8 and 3.7.

This release provides a numnber of reporting improvements to the summary pages and the download functions. These changes were somewhat funded by the Indiegogo campaign. The bulk of financial support came from USPHSCC and Remote Learner.

This release provides the first release of the Moodle Mobile questionnaire app. The mobile app provides all completion functions of the web based app, and allows individual review of completed questionnaires. It does not include the feedback reporting options.
